Is in-home care cheaper than a personal care home in Pennsylvania?

Only at lower hours: at Pennsylvania's 2025 median of about $34 an hour for a non-medical caregiver, 44 hours a week costs about $6,483 a month, slightly above the $6,352 monthly median for assisted living, and around-the-clock home care costs several times more.

Below roughly 40 hours a week, care at home usually costs less than a residential setting, especially if the house is paid off.

What programs help someone stay home?

OPTIONS (60 and older, sliding co-pay, through the Area Agency on Aging), Community HealthChoices (Medicaid, nursing-facility level of care) and LIFE (55 and older, nursing-facility level of care) all pay for help at home, and Medicare covers intermittent skilled home health.

None of these pays for 24-hour supervision for everyone, which is usually the need that drives a move.

When does a personal care home make more sense?

A personal care home tends to make more sense once someone needs help at unpredictable hours, at night, or with medications several times a day, or when the family caregiver can no longer cover the gaps.

Safety events, such as falls when alone or missed medications, are often the turning point.

How do you compare the two fairly?

Compare the full monthly cost of staying home, including care hours, housing costs, utilities, food and home modifications, against a personal care home's written fee at the parent's assessed care level, then add the unpaid family hours each option requires.

Our advisors can help build that comparison with real quotes from homes near you.

What hidden costs should you include when comparing home care with a personal care home?

At home, add grab bars and ramps, a medical alert system, higher utility and food bills, and backup coverage when an aide calls off; in a personal care home, add care-level fees above the base rate, medication management charges, and annual rate increases, which the contract must give 30 days notice for in an assisted living residence.

Also count the family's unpaid hours. A caregiver covering nights and weekends is part of the cost of staying home even when no one sends a bill.
